Job Description



General Summary

Under general supervision, visits individuals and families in their homes, in shelters or other similar places to provide basic community outreach services and to assist with programmatic interventions necessary in coping with the transition from hospital to community, or reducing the risk of being admitted to the hospital. Help to train, mentor, and provide direction to CHW I when assigned and applicable. Assist with special project and training new potential Community Health Worker hires.

Principal Responsibilities and Tasks

The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. These are not to be construed as an exhaustive list of all job duties performed by personnel so classified.

  1. Conduct home visits to validate program interventions and communicates findings routinely to care coordinator team members.
  2. Obtain routine information and reports to licensed team members as directed.
  3. Complete documentation in database and other systems regarding patient or role specific activities.
  4. Assist with the identification of community resources on an ongoing basis.
  5. Communicate general information to individuals and families on program objectives and services, eligibility requirements and benefits, confidentiality of information, etc. Distributes informational materials and literature.
  6. Remind patients and attend scheduled medical or community resource appointments as needed. 
  7. Participate in identified Transitional Care program meetings, and provides patient information regarding status of plan.
  8. Communicate changes to schedules, application processes, co-payments required, and pertinent insurance information.
  9. Prepare written reports and maintain records of outreach contacts and activities.
  10. Update information in database from various data forms collected.

The following statements describe the general nature of work as pertains to training and mentoring CHW Is and potential new CHW hires

  1. Collaborate with the Baltimore Area Health Education Center and and other community and educational partners in the development/revision, planning, implementation, and evaluation of the CHW training program.
    1. Provides classroom instruction.
    2. Helps to prepares all training materials.
    3. Provides guidance and support to community health workers.
  2. Remains current on the learning needs of participants and organizational goals.
    1. Utilizes a variety of assessment methods to determine learning needs of community health workers.
    2. Helps develop educational programs to meet specific needs based on assessment.
    3. Integrates on work and real life scenarios into educational programs.
    4. Help to develops innovative and creative approaches to training which promote other experts’ involvement and facilitates attendance and participation.
  3. Help to develop CHW student field policies and procedures
    1. Work with trainees to develop a “capstone” experience designed for the student to apply and extend learning from the rest of the training curriculum. 
    2. Work with trainees and CHW Is to identify potential goals, skills, activities, and priorities they want to focus on at start of employment.
    3. Work with other employed CHW mentors on how to successfully meet learning objectives of capstone or field experience
  4. Work with current UMMS CHWs and other educator colleagues to improve quality of services and mentor on new role and responsibilities.
    1. Understand insurance process and referral requirements to community resources to help patients,
    2. Oversee updated resource manuals for CHWs
    3. Oversee onboarding and orientation for new CHWs in UMMC
    4. Engage with other community organizations and other staff to improve attitude toward health in the community, and connection between CHWs to the community and other medical providers.

Qualifications



Education and Experience

  1. High School Diploma is required. Associates Degree is preferred.
  2. Community Healthcare Worker Certification is required.
  3. Minimum 2 years field experience.
  4. Minimum 1 year of health training experience.

Knowledge, Skills and Abilities

  1. Demonstrated computer skills including Microsoft office. Ability to effectively operate multi-line telephone console, and to use professional telephone etiquette is required.
  2. Highly effective written and verbal communication skills including courtesy, resourcefulness and efficiency in answering questions, giving directions, and explaining community or department procedures and processes are necessary.
  3. Self-motivated, and confident worker.
  4. Highly effective critical thinking and interpersonal skills, with the ability to identify barriers and promote desired outcomes.
  5. Excellent organizational and work management skills a must. 
  6. Ability to adapt to change.
  7. Ability to multitask at any given time while maintaining strong attention to detail.
  8. Ability to work in a productivity driven environment a must.

  • Pay Range: $22.57-$31.59
